Insurance & Rates
Nutrition DeJunked is in-network with the following insurance companies:
- In-network Insurance:
- We are in-network with the above listed insurance companies. We are also in-network with TRICARE West/TriWest. Many health insurance plans cover nutrition visits with us, often at no cost to you, but coverage varies greatly between plans. We do not check benefits for clients, so it is your responsibility to know what your plan covers and what your portion of the cost is. Before your first appointment, please call your insurance company and inquire about your insurance benefits utilizing our script below. If insurance declines to cover your visit(s) for any reason, you are responsible for the full fee for the service(s) rendered.
- Out-of-network Insurance:
- If we are not in-network with your insurance, we can provide a superbill that you can submit directly to your insurance company. If a superbill is requested, clients are responsible for privately paying at the time of the service. Additionally, submitting a superbill does not guarantee reimbursement to cover the services provided.
- Medicare:
- We are in-network with Medicare, but at this time, Medicare only covers nutrition therapy for people with Type 2 Diabetes or Chronic Kidney Disease Stage 3 or above. If you identify with either of those diagnoses, then we will need a referral from your physician prior to your first session to bill insurance. If you do not identify with either of those diagnoses, then you will need to pay out of pocket for our sessions.
- Medicaid:
- We are not in-network with Medicaid at this time. If you have Medicaid, please inquire about our reduced fee rate and private pay services. Please indicate your interest in a reduced fee rate on the inquiry form. Our priority is to make care accessible for you.
- Out of Pocket Fees:
- If not covered by or not using insurance, our out of pocket fees are $170 for initial sessions and $140 for follow-up sessions. Check, cash, debit/credit cards, and HSA/FSA cards are all accepted as a form of payment.
- Reduced Fee Slots:
- We believe that nutrition counseling should be accessible to everyone, so we offer a limited number of reduced fee slots for people experiencing financial hardship. Please disclose you are seeking a reduced fee slot when you reach out and we will let you know if one is available. Our reduced fee offering is $70 per session.
- Good Faith Estimates:
- Transparency around health care costs is important to us. We will provide Good Faith Estimates under the No Surprises Act explaining the total expected cost of any non-emergency items or services, which you can read more about here. You may ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
